- First and last names of each guest must be included in the submission. Changes to the list must be completed by phoning the specific hotel and club manager.
- This form must be submitted no later than 6:00 PM of the arrival date.
- Contact will be made via phone or email by onsite management to confirm details and determine whether access to club is required for decorations.
- All attendees must be at least 18 years of age with valid photo i.d. and arrive before 10:30 p.m. on your chosen night*.

- ADMIT FREE - V.I.P. is your guest list consisting of you and three friends.
- Individual names required if arrivals are varied.
- If coming together, only the individual making the reservation need be listed and will have +3 next to your name at the door.
